It can be made with any mace weapon or staff class weapon but maces. Flails are more popular because they can be held by one hand, allowing you to carry the shield with the other hand. If you play as a Sorceress, you might want to make a HOTO staff and find a 4-socket staff with a set of ideal modifiers.

The base weapon of HOTO must have 4 sockets to spell out the runeword "KoVexPulThul". You need to add the runes to the weapon in the same order, otherwise, you will not be able to get the Heart of the Oak. So add Ko rune, Vex rune, Pul rune and then Thul rune.

You can find a 4-socket flail anywhere in Act IV Normal, or you can find a normal flail and add sockets using Horadric Cube, only flails are found in Act IV Normal, or you will have a chance to get 4 sockets later, or use Larzuk after you complete Siege of Harrogath quest in Act V.

* Thul is the easiest to find in the HOTO runes, it starts to drop o the Act IV on Normal difficulty.

* Ko followed closely, it started to drop in Act II on Nightmare difficulty, but in the Hellforge on Nightmare or Hell, you have a higher chance to get it, or get it from The Countess on Hell.

* Pul only starts to drop in Act IV on Nightmare

* Vex is difficult to obtain, you can only expect to drop randomly somewhere on Hell difficulty.

Once you make a HOTO flail, you will get the following rewards by wielding it:

* +3 To All Skills

* +40% Faster Cast Rate

* +75% Damage To Demons

* +100 To Attack Rating Against Demons

* Adds 3-14 Cold Damage, 3 sec. Duration (Normal)

* 7% Mana Stolen Per Hit

* +10 To Dexterity

* Replenish Life +20

* Increase Maximum Mana 15%

* All Resistances +30-40

* Level 4 Oak Sage (25 Charges)

* Level 14 Raven (60 Charges)
